Visiting Us

Our practice makes every effort to ensure that your visit to our office is comfortable and pleasant. For your convenience, the building’s main entrance is covered and wheelchair accessible.

New Patients
If you are seeing Dr. Mihelic for the first time, we ask that you arrive 15 minutes before your scheduled appointment to allow time to complete the required paperwork.

If you wish to save time, you can complete new patient paperwork before your office visit. Download, print, and complete the forms below and bring them with you to give to our staff upon arrival:

Late/No-Show Policies
If you arrive late to your appointment, the staff will work you in at our earliest convenience, but we unfortunately cannot hold your timeslot.

If you are unable to keep your appointment, kindly give 24 hours’ notice. Less than 24 hours’ cancellation notice or failure to show up will result in a $25 fee. This fee may be waived at the discretion of the staff for extenuating circumstances.

Financial Policies
ALL patients: please be aware that our practice requires you to pay your deductible and co-insurance in full at the time of your visit. Any refund due to you will be paid promptly should any of it be paid by your insurance carrier.

*We also collect the non-covered amount for Medicare at time of service and will refund promptly if your secondary insurance carrier pays us instead of the patient.

For non-insured patients, we will be happy to work with you on a payment plan.
